Community Health Center
The Community Health Center affords access to medical services for those individuals who may be challenged with little or no income or lack of insurance. By providing affordable care to the families in our community, this program eliminates a common barrier of limited access to care.
Our facility offers comprehensive health services including: prescription drug assistance, physical exams, diagnostic imaging services and gynecological exams.
The Community Health Center is supported by La Porte Regional Health System and grants through the La Porte Hospital Foundation and other supporters.
To be eligible for services, individuals and families must reside in La Porte County. (Michigan City residents are not eligible for Community Health Center services but may be eligible for services provided by The Open Door Health Center, 301 Homer Street Michigan

All Community Health Center patients will be evaluated to determine their service qualification. Qualification and financial eligibility is based on household size and income.
Operating hours are
Mondays & Tuesdays, 8:00 a.m - 5:00 p.m.; Wednesdays, 8:00 a.m. - 8:00 p.m.; Thursdays, 8:00 a.m. - 7:00 p.m.; Fridays 8:00 a.m. - 12:00 p.m. The office is closed from 12:00 - 1:00 p.m. each day

La Porte County Dental Services
La Porte County Dental Services offers access to dental services for those individuals who may be challenged with little or no income and lack of dental insurance. La Porte Regional Health System works to assure that no family in our community will go without quality dental services.
Our services are provided by Robert Grace, DDS, and Ross Lawrence, DDS.
Our facility offers comprehensive dental services including:
· Dental exams
· Teeth cleaning
· Fillings
· Tooth extraction
· X-rays
· Dentures and partials at a reduced fee
La Porte County Dental Services is supported by La Porte Regional Health System and grants through Indiana State Department of Health, the La Porte Hospital Foundation and other supporters.
All La Porte County Dental Service patients will be evaluated to determine eligibility for services. Qualifications and financial eligibility are based on household size and income and follow the federal poverty guidelines. Fees are assessed on a sliding fee scale. Medicaid is also accepted by the clinic for dental services. All patients must reside in
La Porte County.
The La Porte County Dental Services office is located in the Community Health Center building at 400 Teegarden St. (Corner of Washington and Teegarden) in La Porte.

Mondays, Tuesdays, Thursdays & Fridays, 7:30 a.m. - 3:00 p.m.; Wednesdays, 7:30 a.m. - 7:00 p.m. The office is closed from 12:00 - 1:00 p.m. each day.
